Report Medical Student Mistreatment
The College of Medicine is committed to providing a safe and effective learning environment that is conducive to learning by actively encouraging mutually respectful relationships that are free of mistreatment, harassment and threat of retaliation (see the UI COM Positive Learning Environment Policy). If you have experienced or witnessed mistreatment of any kind, or think you may have experienced it, help is available. There are avenues available to you for reporting mistreatment and resources to provide you with support.
The form below may be used to report an incident of mistreatment you have experienced or observed. Reports submitted through this form will be reviewed within 2 business days to determine next steps. As such, you should not use this form to report events presenting an immediate threat to the health or safety of any person, including yourself. If there is an immediate risk to health or safety of any person, please call 911 to connect with medical support or local law enforcement.

Any person may report mistreatment of students using this form. This report form will be received and reviewed by the UI COM Director of Medical Student Learning Environment (DMSLE). Though all efforts will be made to respect the privacy of the person making the report, the DMSLE may be required by law or policy to share the report or information from the report with others. For example, any reports involving Sexual Misconduct as defined by the Sexual Misconduct Policy, must be shared with the UIC Title IX Coordinator pursuant to federal and state law. There are, however, confidential resources available on campus, including the UIC Counseling Center and the Campus Advocacy Network.
