AY2021-22 Appointment and Promotion Forms for the Clinical & Adjunct Tracks

Use the promotion forms posted here and NOT the campus forms available at the Vice Provost for Faculty Affairs’ website.  The College of Medicine’s forms have been customized for the Clinical and Adjunct tracks.


GENERAL INFORMATION


REMINDER: Departments must provide their list of anticipated AY21-22 promotion cases to COM Office of Faculty Affairs by Friday, May 21,  2021.

  • Paperwork due for salaried promotions from Assistant Professor to Associate Professor and from Associate Professor to Professor:  August 2, 2021

SUBMISSION REQUIREMENTS: 

Salaried promotion packets should be submitted electronically via email or shared through Box to [email protected] and [email protected].

For the AY21-22 promotions cycle, affixing scanned signature images is a suitable alternative to securing original signatures.


NON-SALARIED PROMOTIONS

Departments are asked to send their list of anticipated non-salaried promotion cases for AY21-22 to COM Office of Faculty Affairs by Friday, January 7,  2022.

SUBMISSION REQUIREMENTS: 

Non-salaried promotion packets should be submitted electronically via email or shared through Box to [email protected] and [email protected].

Affixing scanned signature images is a suitable alternative to securing original signatures.


NEW APPOINTMENTS

New appointments at the rank of associate professor or professor in the clinical and adjunct tracks may be submitted at any point in the academic year for review at the next scheduled committee meeting. The appropriate forms should be selected from the above options, based upon whether or not the candidate will be salaried.
